OTHER INFORMATION

To enter Kindergarten, students must be 5 years old on or before September 1, 2008.  First graders must be 6 years old on or before September 1, 2008.
The Board of Education of District 86 has recently passed a new Board Policy, which requires students entering Kindergarten or First Grade for the first time, and Sixth Grade, to have a completed physical examination and updated immunizations, including Hepatitis B shots for 5th graders, and the chicken pox vaccine for Kindergarten and Early Childhood, prior to the first day of attendance. Students who do not meet these requirements will be excluded from school until they are completed.

State law also requires that every child, prior to entering early childhood, kindergarten or first grade and sixth grade, have a physical examination and be immunized against diphtheria, pertussis, tetanus, polio, and rubeola (red measles), mumps and rubella (german or three day measles).  *2008-2009 only, students who were enrolled in District 86 during the 07-08 school year and had a 5th grade physical will not be required to get a physical for 6th grade.

Exemptions are allowed only for the following reasons: (1) religious grounds, (2) medical grounds.  Both exemptions require a written signed statement by the parents or legal guardian and a physician’s statement is required for medical grounds.  The grounds for a religious objection must be written in detail and founded on a religious tenet or practice.  Physical examinations and immunizations must be recorded on the form prescribed for statewide use by the Department of Public Health and the Illinois State Board of Education.  Immunizations must be recorded on the form with the month, day and year and verified by the physician.

All Kindergarten, Second and Sixth grade students are required to have proof of a Dental examinations on file.

*NEW LAW – EFFECTIVE JANUARY 1, 2008 requires comprehensive eye exams for children entering kindergarten or enrolling for the first time in public, private, or parochial elementary schools in Illinois.  Children will be required to have this eye exam performed only by qualified eye doctors – such as optometrist and ophthalmologist.  The law calls for the eye exam to take place within one year prior to kindergarteners starting school in the fall and for all students who are entering school for the first time in Illinois.

Parents may contact their building principal for information beginning August 11, 2008.
